    Your Mental Health Navigation will start with a simple assessment to understand your needs and concerns. From there, they'll work with you to explore the best options, guide you to the right care or offer short-term tailored support to help you feel more confident and understood.

  • How we support you

    Once we get your referral, we'll be in touch to chat about how we can support you best. Our Mental Health Navigators may offer appointments by phone, face to face or a mix of both. Your Navigator will guide the process to make sure everything is delivered in the most effective way for your needs, tailored to your circumstances.

  • Who can we support​

    Our Mental Health Navigators are here to support customers who live in one of our homes and are registered with a Wakefield GP. If you're not registered yet don't worry, our Wellbeing Caseworkers will still make sure you get the support that's right for you.

Safeguarding your health and wellbeing

Sometimes, to keep you or someone else safe, we may need to share information with safeguarding services especially if there’s an immediate risk of harm. If that ever happens, we’ll always aim to explain what we’re doing and why, so you’re kept informed and supported throughout.
