Social prescribing connects you to local and national services that support your wellbeing through exercise groups, mental health resources, volunteering opportunities and advice on housing or finances. 

It's all about tackling the social and emotional things that affect your health, so you feel more connected, confident and in control. 

We provide one-to-one social prescribing support for customers living in one of our homes. 

Our wellbeing caseworkers follow the principles of social prescribing, connecting you with local and national services that can boost your wellbeing. They're here to help you explore the social and emotional factors that may be affecting your health, so you can feel more connected, confident and in control. 

We may offer appointments over the phone, meet face to face or use a mix of both. Your caseworker will guide the process to make sure everything is delivered in the most effective way for your needs, tailored to your circumstances and what matters most in your life.

How we provide support? 

Once you've been referred, a colleague will get in touch with you to chat about your needs and what matters most to you. This conversation may happen: 

  • Over the phone
  • Face-to-face in the community 
  • At one of our Hubs 
  • In the comfort of your home 

To get support from us you'll need to be 18 or over, live in one of our homes and have an unmet support need.

With the help of a social prescribing tool, we'll explore key areas of your life and put together a tailored plan that may include: 

  • Your needs, preferences and goals 
  • A clear step-by-step action plan 
  • Links to local services from community groups and charities to national support.

Safeguarding your health and wellbeing

Sometimes, to keep you or someone else safe, we may need to share information with safeguarding services especially if there’s an immediate risk of harm. If that ever happens, we’ll always aim to explain what we’re doing and why, so you’re kept informed and supported throughout.
